For our customers that would prefer to fill out a paper form, all forms are available below.
Forms require a signature. If you do not have a way to print, sign and scan your completed, signed forms, please mail them to us, drop them in our night drop on the front right of our building by the alley, or fax them to us at 712-664-2083. If you can print and scan your signed forms, you can upload them using the “Submit Your Forms” option on the bottom of this page.
For our customers that are filling out paper forms and would like to sign up for auto pay, please also fill out our Automatic Payment Authorization Form for automatic withdrawal from your checking account or Credit/Debit Card.
